Includes a comprehensive 60–90 minute in-home or virtual evaluation, review of medical history, assessment of physical function and daily activities, home safety assessment, goal setting, and development of a personalized care plan.

What to Expect *
An initial occupational therapy evaluation is the first step toward developing a personalized plan to support health, independence, and daily function. Evaluations typically last 60–90 minutes and take place in the comfort of the client’s home—either in person or virtually.
During the evaluation, the therapist will:
• Review Medical History & Lifestyle
Gain a full understanding of current and past health conditions, surgeries (such as c-sections), medications, and lifestyle habits to identify factors that may impact daily functioning.
• Assess Daily Routines & Activities
Explore how everyday tasks are being managed, including self-care, caregiving responsibilities, mobility, home management, rest, and community involvement.
• Evaluate Physical Abilities
Observe and assess key areas such as strength, balance, coordination, endurance, and range of motion during relevant daily tasks.
• Assess Functional Performance
Identify challenges in performing activities of daily living (ADLs) like bathing, dressing, and grooming, as well as instrumental activities (IADLs) such as cooking, laundry, and medication management.
• Conduct a Home Safety & Accessibility Review
Evaluate the home environment for potential safety risks or barriers to independence—particularly important for those recovering from childbirth or seeking to age safely in place.
• Discuss Pain, Fatigue & Emotional Well-being
Consider how physical discomfort, fatigue, or emotional stress may be affecting quality of life, sleep, and energy levels.
• Set Individualized Goals
Collaborate with the client to establish meaningful, realistic goals that reflect personal priorities and needs.
• Create a Personalized Plan of Care
Develop a tailored plan that may include recommended treatment frequency, home exercise programs, environmental modifications, and strategies to enhance recovery, function, and overall wellness.
